- Initial Assessment (60 Minutes): $150.00
- Initial Follow-Up Session/plan (60 Minutes): $150.00
- Occasional Follow-Up Session (45 Minutes):$110.00
- Weekly Follow-Up Sessions (30 Minutes): $80.00
Special Packages:
Phone and e-mail follow-up sessions can be purchased in advance in hourly increments for $150.00 and used in 20-minute increments
Six – 30-minute follow-up sessions – $380.00. ($470 value) Must be used within 3 months from date of first follow-up session. Includes choice of office, phone and/or email appointments. (May be extended for another six sessions to be used in 4 month’s time)
- Testing of Caloric Needs (Metabolism Testing): $50.00
- Grocery Store/Home/Restaurant Sessions: $200.00

.Please note that the last five minutes of each appointment is reserved for scheduling and business matters.
Payment for appointments is requested in advance of the scheduled appointment-as a deposit to reserve your appointment-by cash, check, Master Card, Visa, or right here via PayPal.
In the case of incremental and package follow-ups, payment is to be made for the full amount of the time contracted for before the first session.
CANCELLATIONS: Should be made by calling 615-297-7888.
Cancellations must be made before 8 a.m. the day prior to your scheduled appointment (more than 24 hours notice), otherwise you will charged the fee for the scheduled appointment.
Exceptions are not generally made, even for illness as time is reserved for you. Allison Nutrition Consultants will make every effort to schedule phone or quick consults in lieu of the missed appointment at no additional charge, other than the scheduled appointment charge.
INSURANCE If you want to file insurance, we will provide you with a signed receipt for your payment, which you may file with your insurance company. It is your responsibility to determine what your insurance will pay (prior to any scheduled treatment.
